Digital health technologies (DHT) are increasingly being utilized in the modern management of diabetes. DHT includes continuous glucose monitoring systems, connected blood glucose monitoring devices, hybrid closed-loop systems, smart insulin pens, telehealth, and smartphone applications.
This article describes barriers to their use and offers recommendations on how to overcome these barriers and expand access to DHT for underserved populations, which would be expected to improve outcomes for people living with diabetes.
The article tends to focus on the current “situation” and experience of minoritized communities in the USA; however, it is likely that the principles are generalizable to other countries and healthcare systems with racial, ethnic, and economic disparities.
P2/27 Line 21 “… we describe …”
Line 49 Do you mean paucity of endocrinologists and clinicians serving these or practicing in these underserved communities?
P.3/27 digital connectivity is unarguably important but, in my judgment, referring to it as a “Sixth Vital Sign” is hyperbole.
P.5 lines 55, 56 I don’t understand this statement. Do you mean the clinician is unwilling to challenge the insurance company or do the extra administrative work to appeal insurance denials?
P.8/27 lines 10-14 Please re-write this sentence
Lines 32,33 “… desirable per-meal glycemic outcomes” It is not clear what this means.
P.12/27 line 43 … improvement in maternal-fetal outcomes …
Line 47 paucity of trials
Figure 3 This figure needs to be redone; lettering overlaps
Is Table 2 necessary? The authors have not provided similar tabular evidence for T1D and T2D.

The revised manuscript is improved.
Although most readers will know what PWD refers to, nonetheless, the abbreviation PWD should be defined.
In the edited version, digital health technology is spelled out in full until P34/70 line 12 when DHT appears for the first time. Later, digital health technology is used again. It would be acceptable to abbreviate throughout the manuscript.
P.40/70 lines 44-48 Please re-write this sentence
P.43/70 lines 25-33 This sentence needs to be re-written
P.44/70 line 16 omit “use” after CGM … in both insulin-treated and ….
