The authors have presented a well written and complete review of both clinical studies and studies in pre-clinical animal models of PCOS that provide evidence for the role of GnRH neuron dysregulation in the pathophysiology of PCOS.

Suggested Revisions:
1) The structure of the pre-clinical review portion of the paper (line 225-369) is very clear with subheadings that nicely organize the different topics. The addition of more subheadings in the portion of the paper the reviews the clinical PCOS literature (“Neuroendocrine dysfunction in PCOS” lines 66-224) would help the flow and organization of this large section of the paper.

2) The authors have a very good explanation of multiple different pre-clinical animal models of PCOS and findings from these experiments that have helped to identify the role of altered GnRH secretion in PCOS. An additional rodent model of PCOS that the authors could also consider including in this summary section is the letrozole treatment model used in some experiments to generate a PCOS phenotype in rodents.

3) Figure 1: There is a lot of important information in this summary figure, but it is difficult to tell from the figure in its current form which components of the cycle presented are the cycle in the hypothalamic-pituitary-ovarian axis during the proposed vicious cycle of PCOS and which are present in non-PCOS patients. Having a two panel figure, one with the cycle from a non-PCOS patient and the second panel presenting the components of the cycle that are altered with PCOS. Additionally, more labels on the anatomy presented in the figure (i.e. the ovary and pituitary) and an explanation of the “+” and “-“ symbols in the figure legend would also be helpful.
Minor Revisions:
1) Line 297-299: Please, clarify this sentence. Is the PCOS phenotype, GABAergic input to GnRH neurons or both reversed after puberty with androgen-receptor blockade?

2) Please insert a citation of the effect of the NK3R agonist senktide on KNDy neurons firing rate in PNA mice (Line 366-3680).
